# Forms

Ask people a few questions after they sign up. Each answer is saved with the person.

## Questions after sign-up

Ask people a few questions right after they join. Each project has one optional form for this.

On any page with the form, hosted or embedded, a new sign-up sees the questions one at a time and can answer or skip each. Answers show on the person in People.

- `GET /projects/:id/follow_up`: get the questions. *analytics*
  - Returns `enabled`, `questions` and `answers` (how many answers people have given).
  - Each question has:
    - `key`: stable. Answers stay attached when you reorder or reword the question.
    - `type`: `short`, `long`, `choice` or `rating` (1 to 5).
    - `label`: the question.
    - `choices`: the options, for `choice`.
    - `required`.
- `PATCH /projects/:id/follow_up`: turn the questions on or off, and replace them. *manage*
  ```
  {"follow_up": {"enabled": true, "questions": [{"type": "short", "label": "What do you use today?"}, {"key": "q_ab12cd34", "type": "choice", "label": "How much would you pay a month?", "choices": ["Nothing", "$5", "$20"]}]}}
  ```
  - Send the whole list each time, with each kept question's `key`. A question without a `key` is new.
  - Up to 10 questions. A label is up to 200 characters. A `choice` question has 2 to 10 choices.
  - Turning it on with no questions asks "What do you use today?".
  - Ask what the founder will act on: what people use today, what they'd pay, how likely they are to use it.
